New Patients

Office Information:

Our hours of operation are Mon-Wed 0900 to 5 pm; Thurs Noon to 7 pm; Fri CLOSEDWe are closed for lunch from Noon to 1 pm Mon-Wed.   For all urgent problems after hours, the physician on call can be paged through the answering service which answers our phones after hours.  For medical emergencies, you must dial “911” or go to your local urgent care or emergency room.  All non-urgent matters (such as prescription refills) will be deferred until the next business day. Routine messages and telephone consults for our physician(s) from patients during the work day will be answered within 24 hours.

Advanced Family Health, PLLC is affiliated with Avista Adventist Hospital at 90 Health Park Dr, Louisville, CO 80027. We utilize the Hospitalist Program to assist in caring for any of our patients that need to be admitted to Avista Hospital.


New Patient Forms:

For your convenience, included below are the new patient forms you will need at your first visit. Alternatively, see PATIENT PORTAL below. To expedite your first appointment, please print these out and fill in the information and bring them with you to your first appointment.







Patient Portal:

The Patient Portal allows you to securely communicate with Dr Bennett, review your labwork and medical record, update your demographic information and your personal and family medical history. If you are an existing patient, please contact Liz at Advanced Family Health to obtain a username and password.

If you are a new patient, you can click on the link below that takes you to our Portal and if you click the link on the bottom right that states: ” Would you like to join our practice? New patients can Pre-Register.” Once you click on this, you will be taken to a demographics page that you will need to fill out. Please note, ensure you provide an accurate (non-work) email address. Our office will receive a request to set up your account and you will be notified via email once this is done so further information can be obtained (ie your medical history and your family history) and you can complete the registration process before you arrive for your first appointment.
